The Merck Patient Assistance Program Helps Those in Need

This private and confidential program provides certain medicines free of charge to eligible individuals, primarily the uninsured who, without our assistance, could not afford needed Merck medicines. Individuals who don't meet the insurance criteria may still qualify for the Merck Patient Assistance Program if they attest that they have special circumstances of financial and medical hardship, and their income meets the program criteria. A single application may provide for up to 1 year of medicine free of charge to eligible individuals and an individual may reapply as many times as needed.

Vaccine Resources For Children & Seniors

Merck will continue to provide discounted pediatric vaccines to the Vaccines for Children (VFC) Program.

The VFC program provides free vaccines to children who are Medicaid eligible, without health insurance, or American Indian/Alaska native and to children with health insurance that does not cover the cost of vaccines, if they receive their immunizations at a Federally Qualified Health Center or rural health clinic.

Ask your health care provider if they participate in the VFC program. Or, contact the VFC coordinator in your state. A list of VFC coordinators can be found at http://www.cdc.gov/vaccines/programs/vfc/contacts-state.html

Children

Pediatric vaccines recommended by the Advisory Committee on Immunization Practices (ACIP) are generally covered through private insurance or the federal VFC program.

Seniors

At Merck, we understand that obtaining the medicines you need can be complicated by health care reimbursement. To make the process easier, we created the Merck Access Program. The Merck Access Program provides free reimbursement support services to help answer questions related to insurance coverage and reimbursement.* The Merck Access Program also refers patients to the patient assistance program, which provides certain Merck medicines free of charge to eligible individuals, primarily the uninsured who, without our assistance, could not afford these needed Merck medicines. If you do not meet the prescription drug coverage criteria, your income meets the program criteria, and there are special circumstances of financial and medical hardship that apply to your situation, you can request that an exception be made for you.

The Merck Patient Assistance Program, Inc., may be able to provide medicines and adult vaccines free of charge through periodic bulk replenishments to eligible facilities that serve a large percentage of low-income, uninsured patients. Types of eligible facilities include outpatient pharmacies of disproportionate share hospitals, nonprofit health care clinics, and community health facilities that have a licensed outpatient central fill pharmacy that is owned and operated by the facility. The eligible facilities, under an Agreement with the Merck Patient Assistance Program, Inc., review eligibility of patients for patient assistance under the program's criteria with replenishment being limited to patients meeting specified income and insurance criteria.

This program is designed to help indigent and uninsured patients. The program offers product replacement for:

• NOXAFIL® (posaconazole) Injection (18 mg/mL) for Intravenous Use
• PRIMAXIN® I.V. (imipenem and cilastatin for injection)
• RECARBRIO™ (imipenem, cilastatin, and relebactam) for injection, for intravenous use
• ZERBAXA™ (ceftolozane and tazobactam) for Injection for Intravenous Use.
